No, I don't, but I also have no medical history or allergies to be concerned of.
I, DO, however recommend that if you do take medications, have a specific medical history, or have drug allergies, to have a list of such.
Speaking as someone who DOES the contacting in an emergency, we generally look at the cellphone / wallet. I look for a "mom"/ "dad"/ or someone who's called a bunch of times in the history, as they tend to be the closest ones.
I keep my cell unlocked.
