In fact, he went on medical leave (for the third time) in January; Tim Cook has been steering the ship for nearly a year.
I've seen nothing (either via mainstream press or via personal experience) to indicate that Tim will do anything other than continue Apple's stellar path. Internally we certainly have faith in him.
I haven't personally used the 4S hardware but iOS 5 is pretty darned cool, and I would encourage people to upgrade.
And hopefully you'll all like the Setup Assistant that lets you register without tethering to iTunes.
