Something may be wrong with clutch:

1. Cable-adjuster at lever is fully extended
2. Cable-adjuster at engine is fully extended
3. Clutch-arm is pulled in disengaged position

This might be in response to broken cable stretched out with only couple strands remaining? If the clutch-arm is in that position without any cable pulling on it, there's something wrong inside, perhaps clutch springs broken or cam on arm is broken. Normal position for clutch arm is 90-degrees towards inside.
