The same thing happened to me on a low speed turn, I stepped down to downshift before the turn, after i came out of the turn, i tried to upshift and noticed that there was nothing there for my foot to hook on, i look down and noticed that the shifter was hanging on, luckily the bolt and washer were still hanging onto the shifter portion.

Now I check it and make sure it's not loose every day before i pull it out of the garage for rides.
