Very difficult to tell what went wrong from here in Ct but I would guess you did not tighten your axle bolt enough and the wheel was moving froward and back with de/acceleration. From looking at the picture you appear to have locked the nuts together and I will guess you adjusted your wheel and the chain was at the appropriate tightness. The only cause I can think of would be not enough torque on the axle nut. There is a slight possibility that the adjuster inside of the swing arm is broken/damaged but this should have been apparent when you replaced the cap and nuts. Also if the read wheel bearings were bad then you should have felt something while riding and even if they were missing, had the axle been properly tightened then the adjuster should not have moved.
