You know most city and county offices will print your birth certificate for you. You may need to go to the county you were born in.
I was born and raised in Ohio, I have changed my name 3xs and no issues with just a photo ID and soc. card. My card does not have my middle name but my id does. I agree with 1994, you are over thinking it. If you are concerned take your W2 or other tax papers.
Proof or residence is your ohio DL, it also covers date of birth, name, legal presence. So, all you need is the SS Card.
