Do you have a cylinder that isn't firing right? That would explain all the symptoms you've listed. Been there, done that. Check the plugs/leads/coils for signs that one cylinder might be failing to fire reliably (charred leads, etc).

Why do you assume that the case would have to be full of fuel for there to be fuel in the exhaust pipe?

Are you sure it's fuel in the pipe as opposed to oil?
