Even with gear, someone that small is going to get A LOT more hurt then an adult (i assume)

Though still shockingly young, that's not a "toddler" at all. Though not on the rider pegs, I am somewhat sure that the adult passenger was driving at first and let her take over simple maneuvers once at speed and was ready to lean over her and take primary control back if needed (hopefully, the kid isn't going to do anything stupid with the pedals!). That is an every day sight here in Asia I was surprised there was only one child on the bike you often see an adult and 3 children on bikes here sometimes a dog as well

She wasn't just on it: She was driving it. At the very least she gave throttle and steering input while the other person gave none (not even braking or shifting).

Yep like I say not unusual for here They put the kids on bikes as soon as they can stand and let them ride them its scary to see at the school where I work you often see 3 or more kids on a bike with one driving no adult.
every week you see some kid come in with bandages covering road rash
