Doing a frame-by-frame, it looks like the strap was still fastened. Evidently not tight enough. Helmet came off back-to-front. Maybe too large in the first place.

I wasn't there that day, was there the day before so I missed this but I know both racers and that is one hell of a scary thing to have happen.

He got off the gas right at the finish line. Its customary to keep on it more like a normal race lap for a while so as to not get an enema from riders behind.
