At first I thought that the part flying through the air was the lead riders helmet, but it was just his top-box.

His helmet flies off too. It's laying on the road on the left side and the oncoming car almost hits it.

In the slo-mo, it looks like the flying top box nails him in the head, knocking the helmet off. That implies the helmet wasn't fastened or didn't fit properly. If you're going to find out that your helmet doesn't stay on in an accident, this is probably the best kind of accident to have (as opposed to one at 60mph with your head hitting and skidding along the ground).

As for the second biker... Not paying attention to stopping traffic (the first guy looks like he stops pretty quickly, close to the car) combined with target fixation?
