Couple of guesses;
Did you adjust the chain slack, remove rear the wheel or change wheel alignment recently? You basically want to check for chain/sprocket alignment too.
Clean and lube your chain and go for a test ride. If it's gone then ride on, if it comes back in less than 200 miles, it's time for a new chain.
